aria-labelledby=\"elementor-tab-title-1181\"><ul>\n<li>This procedure is mainly indicated to study <strong>circadian cycle disturbances, Restless Legs Syndrome<\/strong> (RLS), and to determine the <strong>effect of drugs on sleep<\/strong> or in the case of suspected <strong>paradoxical insomnia<\/strong>.<\/li>\n<li>This ambulatory technique allows <strong>continuous recording of body activity<\/strong> during <strong>periods of 1 or 2 weeks<\/strong>.<\/li>\n<li><strong>Description of the procedure:<\/strong><br \/>In order for the study to be carried out properly, the following is available:<br \/> Outpatient<strong>neurological evaluation<\/strong>.<\/li>\n<\/ul>\n<p>Issuance of a <strong>personalized report<\/strong> and, if required, <strong>communication with the elementor-clearfix\" data-tab=\"2\" role=\"region\" aria-labelledby=\"elementor-tab-title-1182\"><h6><strong>Treatment indicated for patients with advanced Parkinson&#8217;s disease, complicated with motor fluctuations and multiple off periods, and who are not candidates for surgical treatment. <\/strong><strong>who are not candidates for surgical treatment. <\/strong> <\/h6>\n<p>In patients with motor fluctuations who require <strong>many daily subcutaneous injections of apomorphine<\/strong>, the use of a <strong>portable infusion pump<\/strong> is preferable, which allows for 14-16 hours daily or even 24 hours.<\/p>\n<h5><strong>Treatment description:<\/strong><\/h5>\n<ul>\n<li>Initial and daily <strong>neurological evaluation<\/strong> during admission by the Movement Disorders Program. Prior to admission, medical treatment is optimized.<\/li>\n<li>The patient is <strong>admitted to the hospital for 3-4 days<\/strong>.<\/li>\n<li><strong>Continuous neurological care<\/strong>, in presence and localized.<\/li>\n<li>Subcutaneous infusion of Apomorphine at decreasing doses until the appropriate dose is reached in each case.<\/li>\n<li>An evaluation of motor function by means of scales and fluctuation diary by a specialist neurologist is necessary.<\/li>\n<li>Monitoring of the infusion system and possible adverse effects during admission. The patient will also be assessed daily.<br \/>Review of tests and history, treatments used throughout the clinical history and the results obtained.<br \/>Withdrawal of antiepileptic medication.<br \/>Continuous neurological care, in presence and localized.<br \/>Daily information to the patient and family members.<br \/>Specific neuropsychological study.<br \/>Psychiatric interview, if considered indicated.<br \/>Skull MRI with epilepsy protocol (high definition).<br \/>Presentation of the case in joint session with specialists from different areas (neurologists, neuroradiologists, neuropsychologists, psychologists, neurorehabilitators, etc.)<br \/>Management of antiepileptic medication before and during admission.<br \/>Daily review of video-EEG records, including critical records. <\/p>\n<p><strong>Issuance of a personalized report<\/strong> and, if required, communication with the referring physician.<\/p>\n<p>At all times,<strong> the Neurology Department secretary<\/strong>, by telephone or in person,<strong> will schedule and coordinate<\/strong> any <strong>necessary<\/strong> complementary tests or other <strong>appointments<\/strong>.<\/p>\n<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-1185\" class=\"elementor-tab-title\" data-tab=\"5\" role=\"button\" aria-controls=\"elementor-tab-content-1185\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon elementor-accordion-icon-left\" aria-hidden=\"true\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-closed\"><i class=\"fas fa-plus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t<span class=\"elementor-accordion-icon-opened\"><i class=\"fas fa-minus\"><\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-accordion-title\" tabindex=\"0\">Transcranial Doppler<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-1185\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"5\" role=\"region\" aria-labelledby=\"elementor-tab-title-1185\"><ul>\n<li>It is a <strong>non-invasive and innocuous technique<\/strong> that uses <strong>ultrasound<\/strong> to <strong>study cerebral blood flow<\/strong>.<\/li>\n<li>It is performed by placing a <strong>probe over certain areas of the skull<\/strong>, called &#8220;acoustic windows&#8221;, which are highly transparent to ultrasound. One of its main applications is the <strong>study of patients with cerebral ischemia<\/strong>, since it provides rapid and dynamic information on the <strong>presence and location of an occlusion or stenosis in the cerebral arteries<\/strong>. <\/li>\n<li>Other applications are the <strong>early diagnosis of vasospasm<\/strong> associated with <strong>subarachnoid hemorrhage<\/strong>, the study of cerebral circulation in patients with stenosis of the cervical arteries (carotid and vertebral) and the <strong>detection of a communication between the<\/strong> right and left <strong>chambers<\/strong> of the heart (right-left shunt). The electromyogram <strong>records and evaluates the electrical activity of the muscle<\/strong>, both normal and when it is altered by diseases of the nervous system. <\/li>\n<\/ul>\n<h5>How is an EMG performed?<\/h5>\n<ul>\n<li>An <strong>intramuscular needle<\/strong> connected by a cable to the electromyograph is placed to <strong>visualize the activity of the muscle<\/strong> being assessed. In order to determine whether surgery is an appropriate treatment, it is necessary to perform a<strong> study to locate the area where the seizures start<\/strong> in the cerebral cortex, and to assess whether this area can be safely resected. <strong>After the study it is possible to identify the epileptic focus and resect it in 80% of patients<\/strong>. <\/p>\n<h6><strong>Treatment description:<\/strong><\/h6>\n<ul>\n<li><strong> <\/strong>To perform the procedure <strong>the patient must be admitted for surgery<\/strong>.<\/li>\n<li><strong>Neurosurgeon and neurologist plan the position of the deep electrodes<\/strong>. To do so, they perform a <strong>three-dimensional MRI study using specific software<\/strong>. <\/li>\n<li><strong>Under general anesthesia<\/strong>, the neurosurgeon proceeds to the <strong>implantation of the deep electrodes<\/strong> (the insertion is guided by a Leksell stereotaxy arc).<\/li>\n<li>Once<strong> awake, the patient is transferred to the Video-EEG Unit<\/strong>. Here the patient is monitored by a team of nurses specialized in epilepsy who are in charge of the Epilepsy Program.
