It causes fusiform enlargement of the extraocular muscles, which is usually bilateral and occasionally symmetrical.

It may occur in the absence of symptoms or previous history of thyroid disease and even with normal thyroid studies.

It may occur in association with systemic hyperthyroidism, primary hypothyroidism, Hashimoto’s thyroiditis and sometimes in people with normal thyroid function.
